Rett syndroom

Rett syndroom is een zeldzame neurologische ontwikkelingsstoornis die in de meeste gevallen veroorzaakt wordt door een spontane mutatie in het gen dat codeert voor Methyl-CpG-bindend Proteïne 2 (MECP2). Het komt bij meisjes voor in ongeveer 1 op 10-12.000 geboortes en is bij jongens veel zeldzamer. Kenmerkend is de schijnbaar bijna normale ontwikkeling in de eerste maanden na de geboorte, gevolgd door een periode van stagnatie en regressie, met verlies van aangeleerde vaardigheden zoals spraak en zinvol handgebruik en met ontwikkeling van stereotypische handbewegingen. Bijkomende kenmerken kunnen zijn: een verstoord bewegingspatroon en afwijkende spierspanning (dystonie, dyspraxie, ataxie), een verstoorde ademhaling, cardiovasculaire problemen, maag-darm problemen, epilepsie en niet-epileptische aanvallen, skeletafwijkingen zoals vervorming van de wervelkolom (scoliose en/of kyfose) en verminderde botdichtheid, slaapstoornissen, toegenomen prikkelbaarheid en verandering in affect of stemming. Rett syndroom is levenslang en kan de levensduur beperken. Het is geen degeneratieve aandoening; velen leven tot in volwassenheid en tonen daarbij vermogen tot leren en herkrijgen of verkrijgen vaardigheden na de regressiefase. 

Het vermogen, of onvermogen, om succesvol te communiceren met mensen in de omgeving en om onafhankelijk en autonoom te functioneren in de maatschappij heeft grote impact op de kwaliteit van leven en het ervaren welzijn. Van alle aspecten van de aandoening, die mensen met Rett syndroom en hun familie ervaren, wordt beperking in communicatie vaak gezien als éénvan de grootste uitdagingen. 

The ability, or lack of ability, to interact successfully with people around oneself and to function as an independent and autonomous communicator in society has a major impact on quality of life and on participation and well-being. Of all the aspects of Rett syndrome that individuals and families experience, difficulties in communication are often rated as one of the biggest challenges.

Het Rett syndroom is een ontwikkelingsstoornis van het zenuwstelsel die bijna uitsluitend voorkomt bij meisjes. Naast het Down syndroom en Fragiele-X-syndroom is het vermoedelijk de meest voorkomende oorzaak van verstandelijke beperking bij meisjes. De oorzaak is een mutatie in het MECP2 gen, gesitueerd op de lange arm van het X-chromosoom. De prevalentie in Nederland wordt geschat op 1 op 10.000 meisjes. Recentelijk is bij een aantal levende jongens en mannen een MECP2-mutatie aangetoond.

BEGELEIDING EN AANPAK

Het Rett syndroom is tot nog toe vooral bij kinderartsen en kinderneurologen goed bekend. Nu de kinderen die in de jaren tachtig zijn gediagnosticeerd ouder worden, dient zich een probleem aan van kennisoverdracht tussen de pediatrische en de volwassen geneeskunde. Vooral de diagnostiek van en zorg voor oudere vrouwen bij wie het syndroom nog niet is onderkend, en die vaak in instellingen verblijven, is problematisch.

GENETICA

In 1999 werd ontdekt dat mutaties in het MECP2-gen de oorzaak waren van het Rett syndroom. Het MECP2-gen ligt op de lange arm van het X-chromosoom (Xq28) en bestaat uit 4 exonen. De coderende sequenties liggen in exon 2, exon 3 en exon 4. Het gen codeert voor het methyl-CpG-bindend proteïne 2, ook wel het MeCP2-eiwit genaamd. In meer dan 80% van de gevallen zijn MECP2-mutaties geïdentificeerd, tot 90 % in klassiek Rett syndroom en wat minder in de atypische gevallen. In ongeveer 20 % van de gevallen zijn (nog) geen MECP2-mutaties gevonden maar het is zeer onwaarschijnlijk dat het Rett syndroom nog door een ander gen wordt veroorzaakt. Vooral bij de atypische Rett meisjes en vrouwen verwacht men wel dat mutaties in andere genen dan MECP2 zullen gevonden worden.

POLIKLINIEK RETT SYNDROOM IN MAASTRICHT EN VELDHOVEN

In het Maastrichtse klinisch genetisch centrum bestaat al langere tijd een multidisciplinair spreekuur voor specifieke patiëntgroepen. Een nieuwe belangrijke ontwikkeling voor Nederland is de start van de specifieke polikliniek Rett syndroom voor kinderen en volwassenen met het Rett syndroom. Deze polikliniek is mede op verzoek van en in samenwerking met de Nederlandse oudervereniging vanuit het klinisch-genetisch centrum Maastricht opgezet. Naast andere specialisten is hier kinderarts/kinderneuroloog E. Smeets werkzaam, die al vele jaren actief is op het gebied van klinische hulpverlening rond het Rett syndroom. Een aantal malen per jaar kunnen ouders met hun kinderen aan een multidisciplinair team hun vragen voorleggen over diagnostiek, begeleiding en opvang.

RETT EXPERTISE CENTRUM 

In 2011 werd het Rett expertisecentrum in het Maastricht Universitair Medisch Centrum geopend. Het betreft hier een initiatief van het Gouverneur Kremers Centrum. De realisatie kwam tot stand in nauwe samenspraak met de Stichting Terre en de ouders van kinderen met Rett syndroom, verenigd in de Nederlandse Rett Syndroom Vereniging. In het Rett expertisecentrum ligt de nadruk op het diagnostisch hersenstamonderzoek. Dit onderzoek is gericht op het beoordelen van de autonome c.q. zelfsturende functies van de hersenstam bij het Rett syndroom. Het centrum is uniek in Nederland.

ONDERZOEK

Het Rett syndroom is een van de belangrijke aandachtsgebieden binnen het project Veroudering en specifieke syndromen. Het onderzoek is gericht op het karakteriseren van de problemen gerelateerd aan de hoofdkenmerken van het beeld voor fenotype–genotype typering. Een goed begrip van wat er gebeurt met ieder meisje met Rett syndroom is essentieel voor de symptomatische behandeling en de zorgplanning op de lange termijn. Uitgangspunt binnen het project is het leveren van een bijdrage aan de verbetering van de dagelijkse levenskwaliteit van kind en het hele gezin.
